    miscommunication on who’s going to answer an email addressed to the team. I took ownership and communicated clearly that the person involves in the task specific to the email inquiry should answer it as protocol as other team members might not know exactly what the issue is about as they handle different activities from different stakeholders.

    More communication. What went wrong? How? What did it affect? How can we make this right?
    Mistakes happen, but knowledge needs to be gained so they’re not repeated. I am dealing with an issue right now even where some huge costs associated with a service were not communicated to me, and then I was not made aware of the billing for an extended period to rectify the cost issue. I am in the process now of comparing options for better costs and will be addressing the lack of notice to ensure it doesn’t repeat in other venues.

    I was called in to a meeting to discuss my position in a situation. I explained how I completely understand the other persons view but based on the current situation how an exception needed to be made and how this was a onetime request and a process was in place to establish the need for the process going forward.

    Happens occasionally – when I think someone did not hear what I said, i turn it around to I did not effectively communicate, train and correct

    Anytime I find miscommunication, it’s important to me that the person and I meet up together, face to face, to clear the air and create understanding again.
